Abstract

Tardive dyskinesia (TD) is a neurological movement disorder. It is induced by neuroleptic drugs but at least some of these movement are due to dysfunction of frontal lobe in Schizophrenic patients. In this research, we assessed the function of prefrontal area in Schizophrenic patients with and without TD to answer the following questions: Is there any relationship between TD and dysfunction of frontal lobe? Is there any way to recognize patients that are vulnerable to TD and to prevent it> In this research 100 Schizophrenic in-patients were chosen (50 with TD and 50 without it). They were assessed with WCST that is a standard neuropsychologic test for assessment of prefrontal function. Results were statistically analyzed with T-test and Chi-square, SPSS. Patients with TD compared to patients without TD had significantly (P~0) increased errors in WCST that showed dysfunction of frontal lobe in patients with TD. It appears with WCST in early stages of Schizophrenia we can recognize patients with dysfunction of frontal lobe and vulnerable to TD and prevent development and progression of TD with atypical antipsychotics and regular monitoring of abnormal movements in these patients.

Abstract

High prevalence of acute psychotic agitation in psychiatric emergency centers, necessitates finding an medication with less side effects and more efficacy. Concerning the reports about the efficacy of atypical antipsychotics (Even in oral route) in such cases. We decided to examine the efficacy of these drugs in comparison with older ones (Oral Risperidon plus Lorazepam and Haloperidol (IM) plus Lorazepams. The sample population included 60 acute psychotic agitated patients admitted to emergency room of Ebne-Sina psychiatric center. The exclusion criteria were seizure disorder-pregnancy-substance abuse, developmental disorders and other major medical problems. These patients were randomly divided in two groups. The first group was given Haloperidol (5 mg/IM) plus Lorazepam (2 mg/PO) and in the second group, Risperidon (2 mg/PO) plus Lorazepam (2 mg/PO) were administered remission was assessed based on the total score and scores of subclasses of PANSS including: Hostility, incooperativeness hallucinatory behaviors, impulse dyscontrol and excitement on time zero, 30 and 60 minutes. The results were analyzed using the proper test analyzer. Significant decrease in PANSS scores were observed in each group (P<0.001). There was no significant differences in total score and scores of subclasses between 2 groups. 3 patients of each group needed additional dose 1 hour after the first one. The mean time to induce sleep in Haloperidol group was 60 minutes (SD: 33 m) and in the Risperidon group 55 minutes (SD: 34 m). The present study similar to previous ones showed that the efficacy Risperidon plus Lorazepam (PO) in the control psychotic agitation was equal to that of intramuscular Haloperidol plus oral Lorazepam.

Abstract

Background and Objective: Yersinia enterocolitica is a worldwide pathogen belong to genus Yersinia. The association between acute childhood diarrhea and other diseases caused by Yersinia enterocolitica has been established by several researchers. Due to the lack of sufficient information on other pathogenic Yersinia species, this study was done to determine the prevalence and the pattern of antibiotic susceptibility of atypical Yersinia spp isolated from children, less than 14 years old. Methods: This descriptive cross - sectional study was carried on 384 children with diarrhea whom referred to the Tehran children medical center, Tehran, Iran during August 2011 to August 2012. 384 fecal specimens of children were transferd to the laboratory and cold enrichment in alkaline buffer with pH of 7.2 for 21 days. The samples were cultured in Cefsulodin-Irgasan-Novobiocin Agar (CIN) differential media in 7, 14, and 21 days. The identification of Yersinia species were carried out by conventional procedure. Antibiotic susceptibility test to Ciprofloxacin, Co-trimoxazole, Tetracycline, Chloramphenicol, Erythromycin, Ampicillin, Gentamicin, Penicillin were determined by standard disk diffusion method. Results: Out of 384 fecal samples, 3 (0.7%) were infected with Yersinia. Three species of Yersinia were Y.enterocolitica, Y.Kristensenii and Y.frederiksenii. All three strains were sensitive to Cotrimoxazole, Tetracycline, Chloramphenicol, Ciprofloxacin and Gentamicin. Conclusion: This study showed that atypical Yersinia play important role in diarrhea. Therefore, more attention should be noticed to atypical Yersinia species in addition to Yersinia enterocolitica.

Abstract

Background and Objective: Mycoplasma pneumoniae bacteria, is one of the most important factor in causing of respiratory infections. Serological and molecular detection methods have their own limitation. Due to this limitation, the application of these methods in all diagnostic laboratories is not possible. Therefore this study was done to determine the rapid detection of Mycoplasma pneumonia by loop mediated isothermal amplification (LAMP). Methods: In this descriptive laboratory study, nasopharynx samples were collected from 92 patients with atypical pneumonia. DNA sample were extracted by boiling method. Six specific primer pairs were designed for LAMP technique by primer explorer ver 4 software. LAMP product identified by adding SYBR Green. Limit of detection and specificity tests have been done for optimizing LAMP test and optimized test carry out for each sample. Results: The LAMP test was optimized using the large Bst enzyme fragment at 66 degree temperature for 1 hour. The detection limit of the test obtained 1 CFU and the DNA replication does not observed in non of the examined pathogenic factors. Out of 92 clinical samples using LAMP technique, 73 cases were negative (80%) and 19 cases were positive (20%). Conclusion: The loop-mediated isothermal amplification technique is simple, convenient and available method for detection of Mycoplasma pneumoniae.

Abstract

Background and Objective: Yersinia is a gram-negative bacillus that cause diarrhea through consumption of contaminated food and water.  This study was performed to identify the atypical Yersinia virulence markers isolated from children with diarrhea.

Methods: This descriptive cross -sectional study was done on 384 fecal samples of 0- 14 years old children admitted at children medical center from August 2011 to August of 2012. Fecal samples, for the enrichment, after 21 days of incubation in alkaline buffer with pH=7.2 at 4degree C, on days 7, 14 and 21 samples were cultured on CIN agar and Mac agar and then confirm the differentiation atypical Yersinia from other typical Yersinia species from fermentation of different sugars. Isolates were tested for marker of virulence including calcium dependence, auto agglutination, Congo red uptake and binding of crystal violet.

Results: Out of 384 stool samples, 4 (1.04%) were infected with Yersinia (Yersinia frederikseni, Yersinia kristensenii and Yersinia enterocolitica). Out of these three, only two samples in association was positive with virulence markers.

Conclusion: Phenotypic markers can be used to study the properties of phenotypic strains of Yersinia.
