fa مقایسه تزریق داخلی ضایعه گلوکانتیم و کرایوتراپی در لیشمانیوز پوستی پاپولر عمومى General تحقيقي Original Articles زمینه و هدف: لیشمانیوز جلدی یکی از بیماری های اندمیک در ایران می باشد. درمان های سیستمیک و موضعی مختلفی برای بیماری استفاده می شود. جستجو برای یافتن روش موثر، ارزان و با حداقل عوارض جانبی برای این بیماری ادامه دارد. برای این منظور تصمیم گرفته شد مقایسه ای بین روش تزریق داخل ضایعه گلوکانتیم و روش کرایوتراپی در درمان لیشمانیوز پوستی پاپولر انجام شود.روش بررسی: در این کار آزمایی بالینی47 بیمار مبتلا به سالک پاپولر مراجعه کننده به درمانگاه پوست بیمارستان قائم (عج) مشهد مورد ارزیابی قرار گرفتند. همه بیماران اسمیر مستقیم مثبت از نظر جسم لیشمن داشتند. بیماران به صورت تصادفی به دو گروه تقسیم شدند. گروه اول تحت درمان با گلوکانیسم به روش تزریق داخل ضایعه به طور هفتگی و گروه دوم تحت درمان با کرایوتراپی هفته ای یک بار قرار گرفتند که در هر دو گروه دوره درمان5 هفته تکرار شد. هر دو گروه 1.5 ماه پس از آخرین نوبت درمان پیگیری شدند.یافته ها: 38 بیمار مطالعه را کامل نمودند که21 بیماربا 35 ضایعه در گروه تزریق داخل ضایعه گلوکانیسم و 17 بیمار با 36 ضایعه در گروه گلوکانیسم با تزریق داخل ضایعه 37.1 درصد ضایعات و در گروه کرایوتراپی 22.2 درصد ضایعات از نظر بالینی، بهبود کامل یافتند (بهبود 100 درصد ضایعه) که با استفاده از آزمون کای اسکوئر اختلاف آماری معنی داری بین دو روش وجود نداشت.نتیجه گیری:کرایوتراپی در مقایسه با روش تزریق داخل ضایعه گلوکانتیم به همان اندازه موثر و در عین حال ارزان تر و با عوارض جانبی کمتر می باشد. Background&Objective: Cutaneous leishmaniasis is endemic in Iran and there are different systemic and local treatments for this disease. There is continuous investigation for finding the most efficient and economical method with little side effects for the treatment of cutaneous leishemaniasis. For this purpose we performed a comparative study between intralesional glucantime injection and cryotherapy in the treatment of papular cutaneous leishmaniasis. Materials&Methods: In this clinical trial 47 patients with papular cutaneous leishmaniasis refered to dermatology clinic of Ghaem hospital in Mashhad were assayed. All patients had positive direct smears. They divided randomly in two groups. First group treated by weekly intralesional glucantime injection and second group treated by weekly cryotherapy. The length of therapy for two groups was 5 weeks. Two groups were followed for 1.5 months after the last week of the treatment. Results: Thirty eight patients completed the study, 21 patients with 35 lesions and 17 patients with 36 lesions were treated by interalesional injection of glucantime and cryotherapy respectively. Clinically in interalesional glucantime group 37.1% of lesions and in cryotherapy group 22.2% of lesions completely cured. The difference between two groups was not statistically significant using chi-square test.
