fa جداسازی لپتوسپیراهای بیماری‌زا از خون بیماران و تشخیص و تایپینگ آنها به روش PCR-RFLP Isolation of pathogenic leptospires from blood samples of patients by PCR- RFLP method میکروب شناسی Microbiology تحقيقي Original Articles زمینه و هدف : لپتوسپیروز یک بیماری مشترک انسان - حیوان شایع در سراسر جهان به ویژه در مناطق گرمسیری و نیمه‌گرمسیری از جمله در ناحیه جلگه‌ای استان گیلان است که توسط گونه‌های بیماری‌زای لپتوسپیراها به‌وجود می‌آید. تعیین هویت لپتوسپیراها با سروتایپینگ توسط روش MAT  پرهزینه، وقت‌گیر و پیچیده است و روش‌های ملکولی می‌توانند؛ جایگزین شوند. این مطالعه به منظور جداسازی لپتوسپیراهای بیماری‌زا از خون بیماران و تشخیص و تایپینگ آنها به روش PCR-RFLP در استان گیلان انجام شد. روش بررسی : در این مطالعه توصیفی ، بیماران بستری شده در بخش‌های اورژانس، داخلی و عفونی بیمارستان رازی رشت که تشخیص بالینی لپتوسپیروز داشتند؛ از ابتدای اردیبهشت تا پایان مرداد 1387 بررسی شدند. کشت‌های مثبت با روش فنل-کلروفرم مورد استخراج DNA  قرار گرفتند. PCR با استفاده از دو جفت پرایمر G1 و G2 و B64-I و B64-II  انجام شد. محصول PCR حاصل از پرایمرهای G1 و G2 با آنزیم DdeI و محصول PCR حاصل از پرایمرهای  B64-Iو B64-II با آنزیم hinf I برش داده شدند و الکتروفورز گردیدند و الگوی باندی آنها با الگوهای مربوط به سویه‌های استاندارد مقایسه وتعیین هویت شد. یافته‌ها : 65 مورد از مجموع 107 نمونه کشت داده شده؛ مثبت گردیدند. 56 نمونه با پرایمرهای G1 و G2 جواب دادند که به گونه‌های اینتروگانس و بورگ پترسنی متعلق بودند و 9 مورد نیز با پرایمرهای  B64-I و B64-II  پاسخ دادند که همگی به گونه کیرشنری متعلق بودند. نتیجه‌گیری : این مطالعه نشان داد که اکثریت لپتوسپیراها در این منطقه از گونه‌های اینتروگانس و بورگ پترسنی می‌باشند. با توجه به مشکلات متعدد در سروتایپینگ لپتوسپیراها، روش PCR-RFLP برای تعیین هویت و مطالعه ساختارهای یک جمعیت داخل گونه‌ای مفید می‌باشد و استفاده مستقیم از آن برای نمونه‌های بالینی و تشخیص سریع و تعیین هویت ، کاربرد دارد. Background and Objective: Leptospirosis is a worldwide zoonosis that is more common in tropical and semitropical regions and in flat area of Guilan province, nothern Iran, caused by pathogenic leptospires.Serotyping by MAT is complicated, expensive and time cosuming. Molecular typing can be useful alternative. This study was performed to identify the typing of endemic pathogenic leptospires, isolated in Guilan province, by PCR-RFLP. Materials and Methods: In this discriptive study, specimens were taken from patients in Razi hospital, Rasht, Iran ,from April to August 2008. DNA of all positive cultures were extracted by Phenol-Chloroform method. PCR was performed by using two primer sets: B64-I, B64-II for Kirschnery serovars whose PCR product were digested by HinfI for RFLP, and G1,G2 for all other species whose PCR products were digested by DdeI. Band profiles of digested PCR products were compared to band profiles of standard servers to determine species and subspecies. Results: 65 of totally 107 blood cultures were positive. 56 Samples were amplified by G1, G2 included Interrogans and Borgpeterseni and 9 samples were amplified by B64-I, B64-II including Kirschneri species. Conclusion: This study showed that the majority of leptospires species are Interrogans and Borgpeterseni. Regarding to several existing problems in Seortyping of leptospires by MAT, PCR-RFLP can be useful for identifying isolating and studying the serovars in different certain species.
