fa پاسخ‌های شنوایی ساقه مغز در کودکان مبتلا به اتیسم Brainstem auditory evoked potential study in children with autism عمومى General تحقيقي Original Articles زمینه و هدف : اتیسم یک اختلال عصبی رشدی مغز است که در سال‌های اولیه زندگی رخ می‌دهد و با علائمی چون اختلالات کیفی در ارتباط کلامی و غیرکلامی، اختلال در تعاملات اجتماعی، عدم توانایی در ارتباط با دیگران، رفتار کلیشه‌ای، فقدان تماس چشمی و حالات صورتی نامناسب بروز می‌کند. شیوع آن 5-2 در ده هزار کودک بوده و در پسران شایع‌تر است. با توجه به تاخیر رشد زبان و گفتار در این کودکان باید بین اختلال رفتاری، ارتباطی و حسی با کم‌شنوایی حسی عصبی یا انتقالی تشخیص افتراقی صورت گیرد. هدف از این مطالعه بررسی ناهنجاری‌های احنمالی تاثیرگذار بر عصب حلزونی یا مسیر شنوایی در ساقه مغز کودکان مبتلا به اتیسم بود.   روش بررسی : این مطالعه مورد - شاهدی روی 12 کودک مبتلا به اتیسم (گروه مورد) و 12 کودک سالم (گروه شاهد) که به صورت داوطلبانه به مرکز جامع توانبخشی ظفر تهران طی پاییز و زمستان 1384 مراجعه نمودند؛ انجام شد. پاسخ‌های شنیداری ساقه مغز در گروه مورد و شاهد در محدوده سنی 12-3 سال ثبت شد. مقادیر زمان نهفتگی مطلق امواج I، III و V و همچنین فواصل بین موجی I-III، III-V و I-V در کودکان مورد و شاهد مقایسه شد. موج V در تمامی کودکان مورد و شاهد تا سطح آستانه ردیابی شد.   یافته‌ها : زمان تاخیر موج V و فاصله بین قله‌‌ای امواج III-V و I-V در کودکان مبتلا به اتیسم افزایش معنی‌داری را نسبت به گروه شاهد نشان داد (P<0.05).   نتیجه‌گیری: این مطالعه نشان داد که سرعت انتقال عصبی در مسیر شنوایی ساقه مغز کودکان مبتلایان به اتیسم کاهش می‌یابد. صدمه به ساقه مغز قسمتی از اختلال نورولوژیک در کودکان اتیستیک است که موجب تاخیر گفتار و اختلال در رشد اجتماعی و شناختی می‌شود. افزایش زمان تاخیر امواج انتهایی ABR در تشخیص زود به هنگام اتیسم کمک‌کننده می‌باشد. Background and Objective: Autism is a neurodevelopmental disorder of the brain,which occures in first few years of life, and characterized by symptoms such as qualititative impairments in verbal and non verbal communication, reciprocal social interactions, inability to communicate with others, stereotyped patterns of behavior, loss of eye contact and inappropriate facial expressions. Its prevalence is 2-5 in 10000 children and is greater in boys. Due to retarded language development, differential diagnosis other than those used with communication, behavioral and sensorial deficits should be made with Sensorineural or conductive hearing loss. The aim of this study was to determine whether children with autism have abnormalities affecting the cochlear nerve or auditory pathway in brainstem. Materials and Methods: In this case – control study, auditory brainstem responses were recorded from 12 autistic children and 12 normal children aged 3-12 years in rehabilitation Zafar central in Tehran – Iran during 2005. Absolute latency values of waves I, III and V and interpeak latencies of I-III, III-V and I-V were compared with the results of 12 normal children as control group. Results: Wave V could be traced down to 25 dB nHL in both normal and autistic groups. Absolute latency of Wave V and Interpeak latencies of III-V and I-V were significantly prolonged in Autistic children in comparison with control group. Conclusion: This study showed a slowing in nerve conduction in auditory pathway in brainstem of autism patients. The brainstem lesion may be a part of neurological damage in autistic children that accounts for deviant language, cognition and social development.
