fa مقاومت آنتی‌بیوتیکی و شیوع ژن‌های متالوبتالاکتامازهای IMP-1 ، VIM-1 و اینتگرون کلاس یک در ایزوله‌های بالینی سودوموناس آئروژینوزا Antimicrobial resistance and prevalence of Metallo-β-lactamase genes IMP-1, VIM-1 and class 1 integron in clinical isolates of Pseudomonas aeruginosa میکروب شناسی Microbiology تحقيقي Original Articles زمینه و هدف: روند رو به افزایش مقاومت به آنتی‌بیوتیک‌های بتالاکتام، در باکتری فرصت طلب سودوموناس آئروژینوزا یکی از عوامل مهم مرگ و میر ناشی از عفونت‌های بیمارستانی محسوب می‌شود. این مطالعه به منظور تعیین مقاومت آنتی‌بیوتیکی و شیوع ژن‌های متالوبتالاکتامازهای IMP-1 ، VIM-1 و اینتگرون کلاس یک در ایزوله‌های بالینی سودوموناس آئروژینوزا انجام شد. روش بررسی: این مطالعه توصیفی تحلیلی روی 200 جدایه مشکوک به جنس سودوموناس از عفونت‌های خون، ادرار، زخم، چشم و خلط در بیمارستان ارسنجان در استان فارس طی فروردین لغایت شهریور سال 1395 انجام شد. پس از تایید جنس باکتری با تست‌های بیوشیمیایی و روش مولکولی S rRNA16و جداسازی گونه سودوموناس آئروژینوزا با استفاده از پرایمر اختصاصی ژن las I ، حساسیت آنتی‌بیوتیکی با روش دیسک دیفیوژن طبق معیار CLSI تعیین گردید. حضور ژن‌هایIMP (Imipenem)، VIM (Verona integron-encoded metallo-β-lactamase) و Int-1 (اینتگرون کلاس 1) توسط روش PCR بررسی گردید. یافته‌ها: نتایج تست‌های فنوتیپی و ژنوتیپی منجر به جداسازی 107 جدایه سودوموناس آئروژینوزا گردید. جمعیت مورد مطالعه بیشترین مقاومت را با 38/79 درصد به سفپیم و کمترین مقاومت را با 13.08% به توبرامایسین نشان دادند. 10جدایه (9.35%) حامل Int-1، 19 جدایه (17.76%) حامل ژن IMP، 23جدایه (21.5%) حامل ژن VIM، 4 جدایه (3.74%) حامل ژن IMP و Int-1، 11 جدایه (10.28%) حامل ژن VIM و Int-1، 15 جدایه (14.02%) حامل هر دو ژنIMP و VIM، 12 جدایه (11.22%) به‌طور همزمان حامل هر سه ژن بودند. 13 جدایه (12.15%) نیز هیچ یک از ژن‌های را نداشتند. نتیجه‌گیری: نتایج نشان‌دهنده افزایش مقاومت دارویی چندگانه و وجود همزمان یک یا دو ژن IMP ، VIM و Int-1 در سویه‌های سودوموناس آئروژینوزا است. Int-1 قدرت جابجایی ژن‌های مقاومت‌زا را داشته و باعث ایجاد جمعیت‌های مقاوم می‌شود. Background and Objective: The ever-increasing resistance to beta-lactame antibiotic in opportunistic Pseudomonas aeruginosa bacteria considered as one of the important factors of death of hospital-acquired infections. This study was performed for determine the antibiotic resistance and prevalence of IMP-1 and VIM-1 metallo-beta-lactamase and integron class I genes in clinical isolates of Pseudomonas aeruginosa. Methods: In this descriptive-analytical study, 200 Pseudomonas spp. isolates from blood, urine, ulcer, eye and sputum infections were collected from Arsanjan hospital in Fars province in south –west of Iran during April-September 2016. After confirmation genus of bacterial by biochemical and 16S rRNA tests, and isolation of Pseudomonas aeruginosa by specific primer of lasI Gene, antibiotic susceptibility was done according to diffusion disk assay and CLSI procedure, the presence of blaVIM, blaIMP and Int-1 genes were determined by PCR. Results: The results of phenotypic and genotypic tests led to the isolation of 107 isolates of Pseudomonas aeruginosa that the highest resistance with (79.38%) for cefepime and the lowest resistance with (13.08%) for tobramycin. Out of 107 isolates, 10 (9.35%) isolates were carrying class1 Integron,19 (17/76%) isolates carrying IMP gene, 23 (21.5%) isolates carrying VIM gene, 4 (3.74%) isolates carrying IMP gene and integron class1, 11 (10.28%) isolates carrying VIM gene, and class1 intgron, 15 (14.02%) isolates carrying both IMP, VIM and 12 (11.22%) isolates simultaneously were carrying each three genes, VIM, IMP and class1 integron. 13 (12.15%) isolates did not have none of these three genes, VIM, IMP, class1 integron. Conclusion: The results showed increased multidrug resistance and simultaneous presence of one or two IMP, VIM and Int-1 genes in Pseudomonas aeruginosa strains.
